It is with a very heavy heart that we have to inform you of the sad loss of Dr Yusuf Ismail Patel GP Principal and founder of Woodgrange NHS Medical Practice. After a valiant struggle with Covid, Dr Patel finally succumbed to his illness on Monday the 20th of April. This is a tragic loss to all his family, friends, colleagues and patients. The pain is immeasurable. He has touched and enriched many lives and we miss him dearly. We understand that many of you will want to send condolences and best wishes. Living with diabetes is difficult right now, faced with uncertainty that comes with the coronavirus and the recent news reports linking diabetes with an increased risk of dying from COVID-19 it is more important than ever to take control of your condition and access appropriate support when you need it. We have adapted our our services to be able to continue to provide the care you need. All of our diabetes services continue to provide care and support through telephone, virtual and online clinics, with face-to-face care available where appropriate, such as foot care clinics. Home Pack Stay Healthy during Isolation
People who are unable to work more than 7 days due to Coronavirus (Covid-19) can now obtain an Isolation Note through a new online service.
The notes can be accesses through the NHS Website and NHS 111 Online.
After answering a few questions, an Isolation Note will be emailed instantly to the user.
If they don't have an email address, they can have the note sent to a trusted family member or friend, or directly to their employer.
The service can also be used to generate an isolation note on behalf of someone else.
The Isolation Note service has been created by the NHS and the Department for Work and Pensions (DWP)

For patients concerned that they may have coronavirus or may have come into contact with someone who has, the NHS 111 online service has created coronavirus pages that tell patients what they should do based on their particular circumstances.

You can get Advice or Treatment within 4 - 48 Hours (2 working Days)
We have introduced the Online Triage Service. You can get access to medical advice without having to pick up the phone or even come in to the practice.
You can avoid long waiting times on the telephone and beat the queues at the front desk.
Getting Started is Easy..!
Simply click on this Link and in the search bar type the symptoms or query that you have. This will produce a series of choices and forms. Click on the relevant one and submit it.
If you cannot find a suitable option to select for your symptom, then select 'Consult with a Doctor form'.
You will be contacted within 48 Hours (working days) about your symptom or query.
The Doctor will read the submitted form and deal with it within 48 Hours (2 working days).
The Doctor will attempt to call you twice from a WITHELD number. If there is no response then the case will be closed. You will need to resubmit a new form.
- Complex problems will always be booked in to see a Doctor
- Urgent problems will be dealt with on the day by telephone or asking you to come in
- Requests that need telephone advice can be addressed promptly
- Some problems may be more suitably dealt with in a specialist clinic e.g Chronic Disease clinic or pharmacist medication review clinic
If you have difficulty speaking English, then please ensure that you have somebody who can help you translate, nearby. Alternatively you may wish to explain your problem to them. If you give us their phone number, we can speak to them directly.
If you have difficulty in completing the form our reception staff will be able to assist.
To ensure you get the best use out of the Online triage system you must ensure that you are set up to use our Online Services. Please speak to Reception about this.
